Sonic the hedgehog 2 I think I can't even remember the name of the game it's been so long

And there was one game that I absolutely loved but for the life of me I can't remember it's name....The male MC of the game would turn into monsters to fight and was given the nickname polly by one of the characters don't remember why. The female MC of the game used chakrams/rings to fight and that's all I can remember.

If anyone knows this game and can tell me the title I will love you forever.

Threads of Fate (He was nicknames polly because the first form his love interest saw him in was a Pollywag. She always thought he was a pollywag that turned into a boy not the opposite.)
