Basically they're a rough ranking of how strong the characters are
in a competitive format (no items, neutral stages, 1 vs 1 in the case of Smash). For the Smash tier lists, the Smashboards forums consider the votes of prominent players and community members, who themselves base their votes on tournament results and personal opinions.
Tier list are highly susceptible to change, because you always find new things in a game. For instance, the first tier list and final tier list for SSBM are almost nothing alike. This is the first Smash 4 tier list they do, only a year after the game came out, so it's going to change pretty significantly in the coming years
There are several uses for tier lists, the most obvious one being as a snapshot of the current competitive metagame at the time of its release. "At this time, this is roughly what we thought of the game".
Tier list are also a base for discussion. "Why did we rank this character this low? Here are our reasons." "I disagree, here are my reasons." It's not always as polite as this of course, but it results in people wanting to prove what they say, which means people playing more, which means players getting better.
Another use is to get people to work "against" certain characters. Sheik is so dominant! Let's all find ways to shut her down, then. Being at the top of a tier list is like having a bounty on your head, because every player is gonna try to find a way to beat these characters with other, "weaker" characters.
It can also get people to work on "weaker" characters, to prove that they aren't as bad as everybody thinks. Everybody loves an underdog, and so seeing someone take a character that's believed to be weak and seeing them find new tricks, new tech, and beasting on everyone is going to be exciting.
Finally, some tournaments have "low-tier only" side-tournaments for fun, so these lists provide a good way to determine which characters can be used in these events.

I totally agree. The top 15 in that list makes sense but everything beyond that is kinda fucked. Old man Ganon is in my opinion at least one or two ranks higher.

Well, you also have to consider his high knockback and how fast his moves are. Almost all of his aerials autocancel if done properly, which means he has almost no landing lag. His limit system is also pretty strong. I'm not surprised, but I won't be surprised if he drops by a rank on the next tier list in a year or so!
