It kind of makes sense though, when the Hollow disappeared as soon as the Old Man basically overloaded Ichigo's powers and promised he would never get them back. Even when he started to get them back from the Fullbringer arc forward, there was no sign of the hollow, only a rework of Old Man Zangetsu. He didn't have access to his real zanpakutou, he didn't have access to the Hollow. Which probably didn't bother the Old Man too much, seeing as how the Hollow and the Old Man never seemed to work together, even when they had the same goal (keep Ichigo alive through the next fight), and the Hollow was very argumentative about being "Zangetsu", which he probably just went with because he didn't officially have a name himself, and Ichigo already recognized the name Zangetsu. Also, calling back to the King and His Horse chapter, the Hollow rebelled until Ichigo showed dominance over it the same way that all the blank zanpakutou did. Granted, I'm not saying this has been planned out from the beginning (at the very least, not in this much detail), but he's probably had some vague idea of this in the works since the Hollow side got a defined identity and wasn't just Nega-Ichigo. Like he probably had the whole "Ichigo's mom was a quincy" idea from near the beginning, because Kubo is apparently way into duality, and he just kind of gradually developed the idea with a few pop flyin' coincidences (The Old Man's appearance versus Juha's) helping it along. I'd imagine that he probably didn't realize the story would get to go past the soul society arc until right around the end of it, and then he seemed to be planning for a big end at Aizen's defeat, but whoops he got to keep going. You really can't have that massive of a story be concretely planned when you don't know when the story is going to end, but that doesn't mean that you can't have several ideas that could play out later.
