What am I supposed to be looking at in the last panel? Neither of Engy's eyes look anything but cartoonish. It's not indicative enough of a punchline.

Not to mention the way Engy has a slanted jaw is really off-model, and it really doesn't look like Engineer if he didn't still have the hardhat and overalls. If I wanted cartoonish exaggerations, I'd expect it from the arms or necks, not the head.

It's a refference to this item (and possibly this one too). The joke is that Engie has more robotic eyes than he knows what to do with, but he hasn't given demo one.

If you're expecting Katie to draw characters completely on-model all the time then you've got the wrong webcomic. Half the humor in her stuff is the exaggerated features. It's a stylistic choice.

Edit idea

Innoculus:
Demoman says "I just miss my eye so much!"
Monoculus appears and kills him.

Also, if the Medigun could restore the scout's broken tooth and other body parts, why couldn't it restore his eye?

In addition, most people actually rely on visual cues (i.e. how large one object appears compared to another) for depth perception more than they rely on the two different images composed from each eye.

There, we have filled the strawman quota for this comic.

I wouldn't say I have the wrong one. Katie seems to be actively going out of her way to draw things as on-model as possible. The only exceptions are when they're deliberately for the sake of jokes, such as the artistic Spy mask strip.

I think It's The Googly Gazer and The Virtual Reality Headset minus the claw thingy.

Plot Twist: Monoculus is Demo's eye.

Also, maybe it did restore his eye and the demo doesn't know because of the eyepatch
