The Sniper in TF2 is generally regarded to use piss as a weapon. The Spy is his natural enemy. Tell me, is it not possible that you could suddenly think of what might happen if there was something wrong with the piss. Blood in the urine is one of the more common signs with bad ailments, that's hardly an original idea.
So, we have the Spy getting Jarate'd, as usual in a TF2 game, except someone then applied that "blood in the urine" idea to suggest Sniper has prostate cancer or something equally horrifying. Or maybe he just peed too much.
As for the Spy's gesture of consolation, what is the common gesture for two men who know each other and one wishes to offer some comfort. A hug is out, and a handshake inconceivable. You guessed it, a hand on the shoulder.
There, I just broke down the entire joke. Can you see now how coincidences can occur through a common thought?
