The Doomsickle from Chrono Trigger, seen here being held by Magus. It's his ultimate weapon, found in a chest in Ozzie's castle. It also raises Magus' attack power when an ally falls in battle.

Green pepper from Mario and Luigi: Superstar Saga. It acts as a stat booster, you pick them up like regular items, and when used in battle, Mario and Luigi toss them into their (or the brother's) mouth with their hands.

Jiggies from Banjo Kazooie. They unlock new levels, and Banjo does hold them in his hand when he gets them.

Ash Crimson's flame from King of Fighters. Usually tossed around as fireballs he can change it shape at will, creating floating orbs, cloaking himself in flames, and other stuff. It's good cuz it hits the other guy and makes you win.

Well, okay. But only because floating orbs and cloak protect you. That's more "a positive effect for you" than hurting the enemy.
