An extreme case and an emphatic reaction; certainly quite unfortunate, but hardly the norm.

Terribly rude, I agree, but smoking has a hobby doesn't necessitate that one needs to actively seek out the maximum amount of people to offend, does it?

I thought Omnithea meant more along the lines that he'd follow around behind said smokers, coughing loudly and intentionally in an attempt to make them go away. Obviously coughing a few times as a bodily reaction can hardly be considered rude, but there are many people who will follow you around intentionally making horrible hacking noises instead of just either walking away or asking you to move, which, really, is hardly polite.
